Alcohol is the most problematic substance among South Dakota residents, with rates of abuse and alcohol use disorders higher than the U.S. national average. Heroin use, while below national rates, has also increased in recent years, and so has marijuana use among adolescents.

After alcohol, the next most common substances people in SD seek treatment (since 2005) for are marijuana and methamphetamines/amphetamines.⁴
